The
Charlotte Junior Soccer application is available in
Adobe
format. To view or download the packet you must have
the Adobe
Acrobat Reader installed on your
computer. This reader is free and available for

Once the reader is installed, follow the link below to
download the packet.
Spring applications will be accepted from January 5
thru February 15th. Spring games begin March 15th.
Fall applications will be accepted from June 1 thru
August 15th.
Please print out the blank application, fill out the
application completely in blue or black pen (remember
to circle desired division of play and GENDER!!!) and
mail it in with your check to CJSF, PO Box 221794,
Charlotte, NC 28222. If you do not circle gender, we
will have to decide based on other clues and we are
inevitably wrong. So please circle gender and fill out
the form completely.
